Before

Initial situation: Caries on 2.4, 2.5, and 2.6.

After

Final situation 20 days after the fillings. Great comfort and no sensitivity at all were reported by the patient.

Diagnosis

  • Clinically, a grey shadow is visible on the distal surfaces of both premolars. 
  • The x-ray confirms the diagnosis of initial caries for both premolars and also for the mesial surface of the first molar. 
  • The patient reports no symptoms.

Treatment Plan

  • Isolation
  • Caries removal
  • Matrix placement
  • Adhesion
  • Centripetal build-up technique
  • Bulk placement of base layer with SDR® flow+ material
  • Occlusal layer with Spectra ST¹ Composite

Clinical Workflow

Step 1: 

The situation after rubber dam isolation.

Step 2:

Initial cavity preparation.

Step 3:

Class II cavity prepared.

Step 4: 

Selective acid etching of enamel with DeTrey® Conditioner 36.

Step 5: 

Etched enamel.

Step 6: 

Effective and fast light curing process with SmartLite® Pro Curing Light, after application of Prime&Bond2.

Step 7: 

 

The complete Palodent3 system in place: sectional matrix, wedge and ring. The flat and compact design of the ring makes stacking of multiple rings possible.

Step 8:

Transformation of Class II into Class I, restoring the interproximal walls with Spectra ST¹ composite in shade A2.

Step 9: 

SDR® flow+ material applied in the maxillary demonstrating great flowability and ability to not flow out of the preparation.

Step 10:

Transformation of a big Class I into a small Class I, using SDR® flow+ material in one increment up to 4mm to create the initial base increment of the restoration. 

Step 11: 

Occlusal surface restored using Spectra ST1 composite in shade A2.

Step 12: 

Finishing with the Enhance® Finishing System.

Step 13: 

Occlusal check.

Step 14: 

Final situation.

Step 15: 

20 days control.
